Application Form Orunodoi 3.0 LPG Subsidy Details
As seen in the application form screenshot, the form mainly has two sections:
1. LPG Connection Details
- Name of LPG Connection Holder in the family
- Father’s / Spouse’s Name
- LPG Connection ID ( Find Your LPG Connection ID –Click Here )
- Aadhaar Number
- Registered Mobile Number
2. Orunodoi Family Details
- Name of Orunodoi Beneficiary
- Orunodoi ID
Consent & Self Declaration
Applicants must sign the self-declaration, confirming that:
- They allow linking of Aadhaar, Ration Card, and LPG Connection ID for the scheme.
- The details provided are true and correct.
👉 The form requires two signatures – one from the Orunodoi applicant and another from the LPG connection holder.
If both are the same person, then the same signature must be made in both spaces.
